Care Coordination seeks to deliver the right care to the right patient at the right time. While it is good for all patients to have coordinated care, it is even more important for those with multiple chronic health conditions. This special service is FREE to eligible children ages 0-16, insured by Medicaid, who are working with at least three specialists to take care of long-term health conditions or disabilities, and our established patients or wish to become a patient at All IN Pediatrics. This program is funded by the state of Indiana and coordinated through Indiana University School of Medicine (IUSM). You do NOT need to change any of your current services or providers – you are not required to receive services from IU Riley Hospital for Children.
What can you expect? You will work with the IUSM team to assess your child and family’s needs and create goals for what you need to achieve the goals you have for your family. Your Special Service Nurse will answer your questions and help you navigate the health care and services system to ensure proper care for your child and a shared plan of care for all of the providers who are involved.
